The aim of the project is to create an innovative model for remote monitoring of chronic diseases - teleassistance / telecare, based on modern information and communication technologies / ICT / in the districts of Vratsa, Vidin and Montana. This will improve the access of vulnerable and lonely people with chronic illnesses and permanent disabilities to occupational health and social care, including those living in remote and small populated areas. The new service will upgrade the activities of the Home Care Centers operating in Vratsa district, and within the project 3 new centers will be established in the municipalities of Vidin, Belogradchik and Montana.

Piloting the teleassistance / telecare service in Bulgaria aims to reach the most vulnerable members of our society and thus, adapted to their needs, allow them to feel safe and stay in their own homes as much as possible. long. The system will provide continuous, automatic and remote monitoring of real-time emergencies and lifestyle changes in order to manage the risks arising from the independent lifestyle of the elderly and other factors such as deteriorating health, lack of adequate medical care and social services in some settlements, etc. In this way, prevention of admission to social institutions and / or frequent hospitalizations of consumers for social reasons will be carried out.

The construction of the teleassistance / telecare system will be done jointly with the project partners and drawing on Norway's experience in this field. The main activities in this direction include construction of a call center in Vratsa, gradual testing of the service in the districts of Vratsa, Vidin and Montana, as well as development of a project for changes in regulations in order to expand and provide this type of services. end of the project.

The beneficiaries of the project will be people aged 50+ with chronic diseases and disabilities, including Roma and representatives of other ethnic communities in the districts of Vratsa, Vidin and Montana. The project also aims to develop effective models for social inclusion and integration of people with low education, the long-term unemployed, incl. representatives of the Roma community by providing vocational training and creating employment opportunities in the field of health and social services.

The project will also create teams of volunteers to support the activities of home care centers and teleservice / telecare services, which includes the development of appropriate training programs and a communication and coordination system.
